Phyllis Ann Chipley Sizemore, 68, of Maineville, Ohio, died Wednesday at Bethesda North Hospital, Cincinnati.
She was a member of Community Family Church, Independence.
Her husband, Gerald Sizemore, died in 1986.
Survivors include a son, Gordon Sizemore of Walton; daughters, Tonya Webster and Melanie Rutherford, both of Walton, and Rebecca Herzner of Maineville, Ohio; 12 grandchildren and 4 great-grandsons.
Services will be at 10:30 a.m. Saturday at Community Family Church, Independence. Visitation will begin there at 9 a.m. Saturday. Burial will be in Floral Hills Cemetery, Taylor Mill.
Memorials are suggested to the Crohn's and Colitis Foundation of America, 386 Park Ave. S., 17th Floor, New York, N.Y. 10016.
Chambers and Grubbs Funeral Home, Florence, is handling the arrangements.
Interment will be at Floral Hills Memorial Gardens, Taylor Mill.
