NEWPORT - Theresa Ann Schlake, 43, died Friday May 30 at St. Elizabeth Hospice, Edgewood.
She was a collections agent for 14 years with the FACS Group Collection Agency, enjoyed reading, walking and family.
Survivors include her husband, Michael Schlake; sons, Joshua M., Eric D. and Chad A. Schlake; mother, Martha Rogers; sisters, Glorial Rogers and Clara Trace; brother, Tommy Rogers Jr.; and four grandchildren.
Visitation will be 11 a.m. - 1 p.m. Tuesday June 3 with services following at Floral Hills Funeral Home, Taylor Mill. Burial will follow in Floral Hills Memorial Gardens.
